Insightly's process to deploy new versions of software on our servers is to do a "rolling deployment" where our applications stay online and available. This happens by deploying our application on a subset of our servers, always leaving most of our systems available to process customer requests. During this planned upgrade, multiple deployments were started in parallel, causing a brief outage of under two minutes of our Web application when all servers were taken offline for upgrades simultaneously.

We've determined the cause of the multiple deployments and have instituted a process to prevent this from occurring again.
